RARE KIDNEY DISEASE SUBSIDY

1. Introduction

The Rare Kidney Disease Subsidy Programme aims to provide financial assistance to Malaysians who face financial barriers in obtaining a diagnosis for rare kidney diseases.
 
The subsidy is intended for low- and middle-income individuals and adopts a universal subsidy approach to expedite diagnostic procedures, without a strict means test at the initial stage. Each subsidy package is capped at RM3,000. Costs exceeding RM3,000 will be subject to means testing, and applicants may be required to co-pay. This subsidy is not available to high-income (T20) individuals who are able to cover their own medical expenses.
 

2. Eligibility

The applicant is subject to the following Terms and Conditions:
• Malaysian citizens;
• Referred by a Nephrologist;
• The diagnostic procedure required, as listed only in the Medical Referral Form, Form A;
• Exclude the high-income strata (T20).

3. Requirement

• The Referring Nephrologist to obtain permission from the patient to register the patient under the Rare Kidney Disease Registry, under the Malaysian Society of Nephrology (MSN).

4. Referral & Submission Process

• Complete the Medical Referral Form A (Nephrologist);
• Quotation (if available) from the Medical Service Provider for the medical screening or procedure.
• Complete the Application Form – Form B (Patient);
• Clear photocopy of the Identity Card of the Applicant,
