CRITERIA FOR ADMISSION INTO NKF

Kidney failure patients who wish to apply for admission into the NKF Haemodialysis Programme must satisfy the following terms and criteria:

  • Applicant is a Malaysian citizen.
  • Applicant is 18 years and above.
  • Applicant is ambulant (able to move about independently).
  • Applicant has suitable functional vascular access.
  • Applicant has no other recourse to regular haemodialysis.
  • Applicant is prepared to appear before the Patient Selection and Welfare Committee of NKF prior to being considered for the programme.
  • Applicant must agree to visits to his / her home by the Welfare Manager / Officer or an Officer nominated by the NKF with a view to verifying all information given.
  • Applicant is prepared to pay treatment fee of RM90.00 per dialysis treatment on admission until the approval of government subsidy of RM100.00 per dialysis treatment is given by the Ministry of Health (MOH). With the approval of the MOH subsidy treatment is free.
  • Applicant is prepared to pay an advance of RM270.00 for a week’s treatment payable on the first session of each week before approval of the MOH subsidy of RM100.00 per dialysis.
  • Applicant may seek assistance from the Centre Manager / Staff Nurse In-Charge of the Dialysis Centre to apply for the government subsidy of RM100.00 from MOH.
  • Applicant is prepared to pay treatment fee as stated above.  This fee is subject to change by the NKF as and when it deems necessary.  This amount is payable prior to treatment.
  • Applicant who fails to obtain the subsidy approval from MOH will have to pay RM90.00 per dialysis treatment while his appeal is referred to MOH for reconsideration. If the appeal to MOH is rejected, the case will be referred to the Patient Selection and Welfare Committee of NKF for its final decision to allow the patient to continue or terminate his dialysis treatment in NKF.
  • The selected patient must be willing to undergo 4 hourly dialysis treatment 3 times a week at NKF Dialysis Centre at the dates and times fixed by the Centre.
  • The patient shall be reviewed by NKF Welfare Manager / Officer as and when necessary at the discretion of NKF with regard to his / her suitability to continue the haemodialysis programme by regular payment of his / her dialysis fees.
  • No visitors are allowed into the Dialysis Centre without the consent of the Centre Manager / Staff Nurse In-Charge of the Centre.
  • The patient must be willing to sign an agreement with the NKF before commencing the dialysis programme.
  • The witness to the patient’s signature should not be the spouse, siblings, children and parents of the patient, or staff and partners of NKF.

Other documents necessary to gain admission into the NKF Haemodialysis Programme are:

  • Complete Medical Report and Laboratory Test results for Hepatitis B, C and HIV of not more than 3 months.
  • 4 photostat copies of patient’s Identity Card – to attach to application form, Agreement, centre file and application for MOH subsidy.
  • 4 passport-sized patient’s photos – to attach to application form, centre file, ID card and Application for MOH subsidy.
  • Latest Income Tax Assessment forms/J forms/EA forms of patient and family members who are working.
  • Latest EPF statements of patient and family members who are working.
  • Latest pay slips of patient and family members who are working or letter of employment or if self-employed, letter confirming income and occupation verified by a Commissioner of Oaths or Penghulu/Ketua Kampung.
  • Latest payment slips of patient or any family member from SOCSO / Pension statements from PSD/JPA.
  • Latest savings/fixed deposit/current account/passbook statements of patient and family members.
  • Water, electricity, telephone and Astro bills from the house where patient and other family members are staying together.
  • Car and house installment receipts of patient and family members who are staying together (attach loan agreement or loan approval letters confirming date of commencement and settlement of loan repayment).
  • Others e.g. insurance premium of patient and family members who are staying together.

Note:  For the application to be processed all documents must be complete. The same documents will be used for the application of subsidy from the Ministry of Health (MOH).

The Patient Selection and Welfare Committee (PSWC) Meeting No. 3 of 2022 on 1 June 2022 reviewed and decided that NKF Patient Eligibility Criteria for Admission into NKF (Paragraph 4.8.1) will be Gross Per Capita income of less than RM1,800/- per head, with consideration of other factors such as:

  • The number of children/dependents who live together and separately;
  • The level of incapacity of the head of the family and family members;
  • The level of support from other family members / income sources, etc.;
  • The condition of home and household appliances, cars, other properties, business, etc. and
  • Proof of financial status and daily expenses of the applicant / family members.
